Each form has an instructions document associated with it. Review the instructions and follow them carefully before submitting any form.

You can submit forms via e-mail or in hardcopy.

Unless stated in the instructions, forms that you submit via e-mail must be sent from your RIT e-mail account. Forms e-mailed from a personal account will be ignored.

If you wish to submit a form in hardcopy, you must bring it in person to the department’s main office. All forms submitted in hard copy must be typed and signed in ink. Handwritten forms will not be accepted.
