Note: The students you are completing evaluations for are only for testing purposes. Also, we ask that you complete the questionnaire by Tuesday, May 11, if possible.

To test our system and evaluate it's usability, please use the following steps:

1. Open this link to the system (opens in a new window). When prompted to log in, use the username and pasword provided in the email you received.
2. Enter the control number for the student you wish to fill out an evaluation form for. Click "Submit."
3. Complete all the fields in the form. Click "Submit."
4. Repeat steps 2 and 3 for the remaining control numbers you have received.
5. Complete this questionnaire (opens in a new window).
